Philosophy:

I work for the University (UCSF Fresno) so I can teach and train the doctors of the future. I am expected to teach the next generation of doctors what is the optimal quality of care based on national guidelines, and I practice what I teach. Because I work for the University, I get a salary, rather than being paid on a "per patient" or "per test" basis like a typical private practitioner. Therefore, there is no incentive for me to run unnecessary yearly tests on you if they aren't warranted.
