Abstract:
A surgical staple, particularly but not exclusively for use in endoscopic or laparoscopic surgery comprises a first leg provided with a first locking element, a second leg provided with a second locking element, and a hinge joining the first leg and the second leg to one another. The first locking element and the second locking element are designed to cooperate with one another to maintain the staple in a closed postfiring configuration. The staple is disposed in a closed prefiring configuration wherein at least one of the first leg, the second leg, the first locking element and the second locking element is deformed to prevent cooperation of the first locking element and the second locking element. The staple legs are provided with latching elements which cooperate with corresponding elements on a forceps jaws to enable a pivoting of the legs alternately away from and towards one another and about the hinge during a staple opening operation. Such latching elements may include loop-shaped eyelets or snap-lock projections such as knobs or cooperating snap-lock-type recesses.

Abstract:
A method for removing a gall bladder comprises the steps of (a) providing a surgical instrument including a flexible loop and a flexible web member connected to said loop to form a capture pocket, the loop defining a mouth opening of the pocket, (b) closing the cystic duct, (c) severing the cystic duct, and (d) passing the loop around a portion of the gall bladder including a closed and severed segment of the cystic duct, thereby enclosing the gall bladder portion in the capture pocket. The method further comprises the steps of (e) severing connective tissue between the gall bladder and the adjacent liver subsequently to the passing of the loop around the bladder, (f) maintaining the gall bladder portion surrounded by the capture pocket during the severing of the connective tissue, and (g) shifting the loop relative to the gall bladder to enclose a greater portion of the gall bladder in the capture pocket upon a severing of some of the connective tissue between the gall bladder and the adjacent liver. The method is adaptable to removing other internal organs, particularly organs which are coupled to adjacent tissue masses via connective tissue.

Abstract:
A device for controlling flow rate in an intravenous line comprises a housing having an inlet port adapted to be connected to an incoming segment of an intravenous line and an outlet port adapted to be connected to an outgoing segment of the intravenous line. A valve is disposed in the housing between the inlet port and the outlet port. The device further comprises a first valve adjustment mechanism mounted to the housing and operatively connected to the valve for setting the valve to select a desired flow rate, the first valve adjustment mechanism being manually adjustable for selecting the desired flow rate. A second valve adjustment mechanism is mounted to the housing and is operatively connected to the valve for automatically and temporarily closing the valve in response to the exceeding of the desired flow rate by an average flow rate of intravenous fluid into the housing through the inlet port.

Abstract:
A medical instrument usable with an ultrasound-endoscope for performing a needle biopsy on a patient's internal body tissues usable at a surgical site not visible to the unaided eye or viewed endoscopically, comprises an elongate tubular member with a hollow needle element connected at its distal end, a sheath member housing said tubular member and needle element, an actuator subassembly with a shifter member operatively connected to elongate tubular member's proximal end, and a distal camming subassembly, said subassembly enabling a rotating motion of said needle member while handle actuator is moved in the forward direction. Upon inserting an ultrasound-endoscope into a patient and locating a mass, the fine needle with sharply pointed spoon shaped distal end is inserted into the mass aided by endoscopic and ultrasonographic guidance. Once in the mass, a camming action is initiated, causing rotation of the fine needle within the mass, resulting in a scooped out core biopsy. This instrument enables the performance of a fine needle aspiration requiring only one or two needle introductions, with a resultant core biopsy substantial enough for diagnostic purposes.

Abstract:
A surgical stapling apparatus comprises an endoscope, an elongate flexible tubular member slidably disposed in a biopsy channel of the endoscope, an elongate flexible rod member slidably disposed inside the tubular member and a staple disposed in a closed configuration inside the endoscope biopsy channel distally of the distal end of the rod member. The staple is releasably connected to the rod member. In operating the stapling apparatus, the endoscope is inserted through an aperture in a patient's body, the endoscope's optics being used to locate a surgical site. Upon finding the surgical site, a surgeon pushes the flexible rod in the distal direction to eject the staple from the endoscope biopsy channel. Upon ejection, the staple is opened, either through the action of its own internal forces or through the action of forceps jaws operated by the surgeon via an additional tubular member slidably disposed in the biopsy channel about the first tubular member. The rod is then pushed further in the distal direction to move the staple towards a blood vessel to be ligated or an opening to be closed.

Abstract:
A surgical instrument assembly for use in snare cauterization operations comprises a tubular sheath member, a metallic cauterization loop, and a metal wire operatively connected to the loop, the wire passing longitudinally through the sheath. An electrical supply is operatively connectable to the wire for feeding an electrical current to the loop via the wire, while a manually actuatable shifter is operatively connected to the wire for longitudinally sliding the wire along the sheath in alternately opposite directions. A flexible web member is connected to the loop to form a capture pocket, the loop defining a mouth opening of the pocket. During use of the snare cauterization instrument, the loop is at least partially expanded from a collapsed configuration and passed over the selected internal body tissues to be removed, so that the web member substantially surrounds the selected internal body tissues. The loop is then at least partially closed to engage the selected internal body tissues around a base region thereof and an electrical current is subsequently conducted through the loop to burn through the selected internal body tissues at the base region thereof, thereby severing the selected internal body tissues at the base region. Upon a completed burning of the loop through the base region of the selected internal body tissues, the loop is at least partially closed, whereby the severed internal body tissues are automatically captured by the web member.

Abstract:
A device utilizable with an endoscope to promote sterility comprises a substantially thin-walled elongate tubular sheath made of a flexible material such as rubber, the sheath having a use configuration with an inner diameter larger than an outer diameter of an insertion member of the endoscope, whereby the sheath may be removably disposed about such insertion member to substantially surround same prior to insertion of the insertion member into a patient.
