I offer home visits to homebound patients (for whom outings are taxing and/or require the assistance of others or specialized equipment) for the purpose of: Transitional care after hospitalization or rehab, Assessment, treatment and counseling in chronic illness and its symptoms to supplement MD visits, Palliative or comfort care, Face-to-Face assessments for the MD to certify home health and DME, Primary care when MD office visits are not feasible. Accepting Medicare and private pay.
