Abstract:
An enclosed guidewire exchange system is described which utilizes a flexible sterile barrier which is preferably tubular and folded or rolled prior to use. The barrier functions to enclose the old catheter as it is being withdrawn from the patient. The system also provides a sterile barrier for enclosure of the guidewire upon removal of the old catheter from over the guidewire. Thus, the system preferably includes a guidewire container containing a flexible guidewire. The container includes a connector which may be a threaded female end for attachment to the pilot tube luer of the distal port of the multilumen catheter or other indwelling catheter. The guidewire container preferably includes an interrupted portion such as a window to allow the physician to contact the guidewire within the container so as to advance the guidewire into catheter once the container has been connected to the luer of the catheter. The sterile barrier enclosure is preferably attached to the guidewire container proximal to the female connector and unfolds or unrolls to provide a sterile cavity distal to the guidewire container. The sterile barrier can include a proximal catheter barrier portion and a distal guidewire barrier portion and a tear point or region intermediate those portions.

A system and method for the automatic diagnosis of obstructive sleep apnea in a centralized hospital critical care monitoring system for the monitoring of a plurality of patients in at least one of a critical care, step down, and cardiac ward by telemetry. The system includes a central processor having a display, and a plurality of telemetry units for mounting with patients, each of the telemetry units has a plurality of sensors for connection with each patient, the telemetry unit is capable of the transmission of multiple signals derived from the sensors to the central processor, in one preferred embodiment the method comprising steps of programming the system to analyze the signals and to automatically identify the presence and severity of obstructive sleep apnea and to provide an indication of the identification.

Abstract:
A medical valve is provided that includes a housing having a main conduit and a branch extending from the main conduit and in fluid communication with it. The bore of the branch has an elastomeric septum piston disposed therewithin and at least a portion of the elastomeric septum piston is movable by either compression or by a longitudinal advancement along the secondary bore towards the bore of the main conduit. In one embodiment, the septum piston is cylindrical and includes a centrally positioned slit. The bore of the branch includes two opposing projecting members along a distal portion of the bore which effectively narrow the diameter of the bore. When the elastomeric member is pushed from a proximal position to a distal position, it is transversely compressed by the projecting members to open the slit and allow fluid passage. After removal of the longitudinal displacement force the septum piston retracts away from the distal portion back to the proximal portion.

Abstract:
A vented fluid collection system including a collection apparatus for use with a device in communication with a mammalian organ or body lumen having a fluid to be sampled therein. The fluid collection apparatus has a blunt cannula having a bore defined therein and a wall defining a small collection chamber in communication with the bore. A fluid sample passes through the bore and is stored in the collection chamber. A vented membrane is disposed at an opening in the wall to allow gas to pass therethrough but prevents fluid collected in the collection chamber from passing therethrough. A shroud surrounds and protects the cannula and a cannula cap may be mounted to the shroud to enclose the cannula. In an alternative design of the fluid collection apparatus, the bore acts as the collection chamber. In all designs, when the fluid collection system communicates with a blood vessel, the patient's systemic blood pressure urges the fluid sample into the bore and, in the first design, further into the collection chamber.

Abstract:
A system for the sequential and repetitive aspiration and injection of fluid. The system includes a syringe having an internal volume, a piston to vary the internal volume and a chamber divider for separating the internal volume into proximal and distal reservoirs. The chamber divider is moveable along the chamber and is positioned adjacent the distal end of the chamber prior to withdrawal of fluid into the syringe. The proximal reservoir has a maximum displacement volume for receiving fluid. A flow channel provides flow between the proximal and distal reservoirs and a valve disables and enables flow therethrough. A tensile element links the piston and the chamber divider so that after the piston is moved and the proximal reservoir is filled with a first liquid, the chamber divider is pulled away from the distal end of the syringe to enlarge the distal reservoir and draw the blood into the distal reservoir. A conduit is connected with the syringe and is in communication with the patient's blood vessel and contains the first liquid. The flow channel and the conduit are sized and configured such that after the maximum volume of first liquid has been withdrawn into the proximal reservoir and the blood has been withdrawn into the distal reservoir, the maximum volume of first liquid is substantially free from blood, and subsequent displacement of the maximum volume of first liquid back through the valve is sufficient to displace all the blood from the syringe when the piston is advanced.

Abstract:
A medication delivery system is disclosed to mitigate or reverse the pharmacologic effect of a medication beyond the desired therapeutic window. The system provides sequential delivery of medications including a first agent which induces a physiologic change, a second agent for reversing the physiologic change, and a medication release delaying agent for delaying the release of the second agent so that, upon receipt of the medication delivery system into the body the first agent is released before the second agent so that the physiologic effect of the first agent is mitigated by the second agent. The system can be a pill with the first agent comprising an outer medication and the second agent an inner medication with the medication release delaying agent intermediate the outer medication and the inner medication. In an example, the first agent can comprise a sleep aid and the second agent an arousal agent.

A displacement-activated medical check valve is provided for selectively opening and closing medicinal fluid communication. The check valve includes a flexible elastomeric member that has a proximal end and a distal end and has a sealed perforation defined at least partially through the flexible elastomeric member. A housing receives the flexible elastomeric member so that it can be displaced along the length thereof. A contact member is provided for engaging the distal end of the elastomeric member when at least a portion of the elastomeric member is displaced against the contact member. The elastomeric member and the sealed perforation are sized and configured such that the perforation is shortened and thereby compressed open when the elastomeric member is compressed against the contact member. When a compressive force is removed the elastomeric member rebounds to close the perforation.
