Abstract:
A method and apparatus for anterior cruciate ligament (ACL) reconstruction using holes in bone created by retrograde cutting. A rotary cutter, mounted onto an insertion post of a tibial guide, is inserted through an anteromedial portal and the rotary cutter placed on the anatomical origin of an ACL tibial insertion. A drill pin is drilled through the tibia and advanced until it contacts and engages a cannulation in the rotary cutter on the guide. Further rotation of the drill pin disengages the rotary cutter from the guide. The retrograde drill pin is then retracted and simultaneously rotated for retrograde cutting of a socket or tunnel of desired depth in the tibia. A strand is passed through the cannulation of the drill pin into the joint. The end of the strand is retrieved from the anteromedial portal and a loop formed for subsequent graft installation. A similar technique is used to form a socket or tunnel in the femur except that a femur guide is used in place of the tibial guide. A graft of appropriate length is pulled through the anteromedial portal by the loops formed in the strands and the ends of the graft are drawn into the femoral and tibial sockets. The reconstruction is completed by securing the graft ends.

Abstract:
Techniques disclosed herein include systems and methods for a bone screw and self-retaining driver. The system includes a mechanism that enables a bone screw to be rigidly attached to a screwdriver for manipulation and transfer of bone fragments and grafts to a fixation site, such as during surgery. The system includes a cannulated bone screw that has a drive connection that engages with a drive structures of a screwdriver. The bone screw also includes a structural connector that securely attaches to or receives an internal shaft or rod of the screwdriver. The internal shaft or rod of the screwdriver can rotate and slide independent of an outer shaft of screwdriver. Accordingly, the internal shaft of the screwdriver can rigidly connect with the bone screw enabling applied torque to the screwdriver to be transferred to the bone screw, without the bone screw falling off of the drive connection.

Abstract:
Endosteal fixation of a ligament graft with an interference fixation device installed in a retrograde manner is disclosed. An interference screw is installed at the top of the tibial tunnel, through the tibial plateau, in a procedure for ACL reconstruction. The interference screw is fitted with a length of suture for pulling the screw onto a driver placed through the tibial tunnel. The suture is secured to the screw using a knot, an adhesive, insert molding, and equivalents. The suture extends beyond the leading tip of the screw a sufficient length to allow the screw to be passed into the joint by pulling on the suture exiting the tibial tunnel. A driver fits into a drive opening in the leading front end of the screw. Turning the driver advances the screw in a retrograde manner into the tunnel. Accordingly, interference fixation of the graft near the tibial plateau is provided, thereby eliminating graft abrasion at the tibial plateau tunnel opening.

Abstract:
A fixation device for securing soft tissue or suture to bone, such as in the case of rotator cuff repair and primary or secondary ACL tibial side fixation, is disclosed. The fixation device includes an implant screw and a washer that attaches to the head of the screw, yet allows the screw to be turned independently of the washer during insertion of the screw. The washer can be a cap-shaped washer, for covering and securing suture knot bundles around the implant screw, or a spiked cap, for engaging soft tissue. If necessary, the length of the implant screw may be cut to adjust its length.

An instrument for retrieving suture within a patient has a shaft with proximal and distal ends. The distal end terminates in an articulating mechanism, such as a jaw that captures suture slidably within an opening disposed toward the instrument tip. An actuating mechanism disposed on a handle proximal to the shaft opens and closes the jaw. The handle is adapted to be gripped in the user's palm, and the actuating mechanism is toggled between two operative positions, preferably by flexion and extension of a single digit. A surgical instrument and method of using same are provided. The instrument includes a jaw portion extending along an axis and a handle portion rotatably connected with the jaw portion and configured to at least partially rotate about the axis with respect to the jaw portion.
