Philosophy:
The critical avenue in my practice is diagnosis. Since this process is so important in determinng a pain generator, my philosophy is to perform the history and physical myself. The patients see me every clinic visit. One of my professors told me that is you listen to a patient long enough, they'll tell you what's wrong, and if you listen to them longer, they tell you how to treat them. Studies have shown that a careful history gives the doctor the diagnosis in 80% of cases. That being said, I rely on a multidisciplinary approach to treatment. I utilize a diagnostic injection specialist, therapists in chiropractic, acupuncture or PT and pain psychology who are skilled in their approach. If surgery is necessary, my patients have the benefit of the correct diagnosis, an opportunity to have attempted all non-operative avenues and to have the most precise and minimally invasive surgical (MIS) strategies available anywhere in the country.

Abstract:
The foregoing application describes a system and method of performing a minimally invasive surgical operation. More specifically, the invention involves the use of disposable cannula and slender dilators of variable lengths, which incorporate a source of illumination to carry light to a surgical site and video capabilities for capturing and displaying images from a CMOS or CCD camera device. According to one embodiment, fiber optics run semi-circumferentially or along walls of the cannula/dilator and terminate at about a centimeter from the distal end of the cannula/dilator, thereby preventing illumination from “bottoming out” at the floor of the incision. According to one alternate embodiment, the light fibers may be fashioned in an annulus around one or more camera chips to provide illumination and video of the surgical site. In still another embodiment, the light fibers may be replaced by light emitting diodes in a more remote light source or alternatively at the distal-tip of the CMOS or CCD camera device.

Abstract:
An apparatus and method of performing a minimally invasive posterior spine fusion. More specifically an apparatus with a handle and a forked head on the distal end of the handle is used to grasp implant material and introduce the material to an implant site. The shaft of the apparatus is shaped so as to allow the affixation of a drill guide and drill while simultaneously holding the implant material in the implant site. After removal of the boring tools and assembly of the fusing element, the apparatus can be selectively removed from the implant site. A method of achieving facet joint fusion with near simultaneous fixation is also disclosed.

A medical retractor apparatus that includes a housing assembly and a retractor assembly is provided. The housing assembly includes a retractor body having a proximal end. The retractor assembly includes a retractor element having a tip. The retractor element is extended/retracted relative to the proximal end. The retractor element is extended offset from the center of the proximal end. A guide member with a slot located at the proximal end prevents unacceptable turning of the retractor element. Preferably, the retractor element includes two spaced loops that can be surrounded by a cover member. The retractor element desirably positions a body vessel of at least about 1 cm in diameter during a surgical procedure. The retractor element has sufficient tensile and compressive strengths to position the great vessel, while avoiding unwanted bulging of the great vessel and visual interference during the surgical procedure.
