A method of determining the size and placement of screws in pedicles during surgery in a selected spinal area, comprising using a computer to generate a dimensionally true three-dimensional image of the bony spine in the selected spinal area; using a computer to hollow out the vertebra in the three-dimensional image with cortical wall thicknesses selected by a surgeon performing the surgery; using a computer to determine the narrowest cross section (isthmus) within each pedicle; using a computer to generate a straight line starting at the center of the isthmus and extending in opposite directions so that it is positioned concentrically within the pedicle without touching the walls thereof, the line terminating inside the vertebral body a predetermined distance from the anterior inner cortical wall and extending outwardly in the opposite direction to penetrate the posterior pedicle cortex; using a computer to expand the line concentrically and radially to a cross sectional size that is less than that of the isthmus, the line being expanded into a cylinder that stops growing when any portion thereof contacts the inner cortical wall of the hollowed out vertebral body, with the exception of the posterior pedicle cortex; and using a computer to calculate the ideal pedicle screw diameter, length and trajectory based on the dimensions and trajectory of the cylinder generated for each pedicle.

A method of determining the pedicle base circumference and the pedicle isthmus to facilitate screw placement in a pedicle of a vertebral body during spinal surgery, comprising providing a series of first lines tangential to the outer cortical surface of the vertebral body in and near the pedicle on a transverse section from a three-dimensional image of the vertebral body, providing a series of second lines extending through the vertebral body in and near the pedicle thereof in perpendicular relation to the series of first lines, identifying the pedicle base circumference as the areas of the outer cortical surface where adjacent second lines are at the greatest angle with respect to one another, and identifying the pedicle isthmus as the areas of the outer cortical surface where the second lines that are opposed to each other are closest to being parallel to one another.

An adjustable awl for forming a hole for the insertion of a screw or other device in a pedicle or other body part. The awl comprises an elongated housing having an open end, and an elongated awl member movably mounted in the housing and being extendable beyond the open end to vary the length of the awl. A method of determining the size and/or placement of a prosthetic disc in a disc space between adjacent endplates of two vertebrae in a selected spinal area, comprising: using a computer to identify the disc space parameters of height, width, depth and lordosis between the adjacent vertebral endplates; using a computer to create an artificial volume corresponding to an actual prosthetic disc that can be positioned in the disc space in accordance with a manufacturer's size and lordosis specifications for prosthesis; using a computer to determine the center of the disc space and the center of rotation of the prosthetic disc volume; using a computer to position the prosthetic disc volume in the disc space such that the center of rotation of the prosthetic disc volume is positioned posterior to the center of the disc space; and using a computer to determine the prosthetic disc volume that fits within the disc space in accordance with a surgeon's or manufacturer's specification for prosthesis.

Abstract:
A method of closing an open wound in a body part, comprising making a plurality of spaced incisions in the skin adjacent to or near the wound sufficient to enable the skin to be stretched over the wound. Thereafter, the skin is stretched over the wound and secured in place over the wound to close it. Also, in accordance with a new method of decompression for fasciotomies, spaced incisions may be made in the skin overlying the muscle compartment to be decompressed to allow for adequate drainage and decompression of the swollen, tense and compromised muscle compartment, and preserve the integrity of the overlying skin to minimize or eliminate the need for skin grafting. A new and improved circular surgical scalpel may be used to make the spaced incisions in the skin.
