Abstract:
A transcatheter occluding intracardiac defect device comprises a folding poly-urethane foam with Teflon coated wire skeleton, introducible into a long vascular sheath and automatically unfolded upon delivery in the distal to the defect cardiac chamber. A two mm string loop is attached to the center of the occluder, while the loop is closed by a 1 mm knot. A loading wire comprises a Teflon hollow wire and a double mono-filament thread which carries the occluder on the one end and is tied on the other end. An occluder-holder comprises a rhomboid poly-urethane foam with Teflon coated wire skeleton and a "rubber" piece sutured in the center. The occluder-holder is advanced over the loading wire axis and through the long sheath in the proximal to the defect cardiac chamber and is pushed against the occluder till it is securely buttoned. The distal end of the loading wire is cut and the hollow wire is pulled out over the double thread. The thread is pulled as a single strand and therefore the device is released.

Abstract:
An intravascular prostheses is delivered transarterially or transvenously to occlude cardiac defects. The defects, which may include the patent ductus arteriosus, the ventricular septal defect and the atrial septal defect. The prostheses is a device having a distal occluder attached to a string and a proximal occluder connected to the string. The occluders are delivered to the heart by known methods. With the distal occluder on the distal side of the defect and the proximal occluder on the proximal side of the defect the occluders are adjusted according to the thickness of the heart structure at the defect. In one embodiment the adjustment is by moving the distal occluder over a series of knots or buttons in the string. In another embodiment the proximal occluder is connected to the distal occluder by an elastic string so that the elastic tension of the strings bring the occluders into position. A radiopaque button is placed upon the string to aid positioning the occluders.

Abstract:
A heart defect in the form of an hole is corrected by placing a patch over the hole. The patch is first attached to an uninflated balloon which is placed distally of the hole. The balloon is inflated with fluid and the patch is positioned upon the hole. After endothelialization the balloon is deflated and removed through a small hole in the patch. The small hole in the patch is closed by a purse string. During endothelialization the balloon is held in place by a disk or second balloon on the proximate side of the defect.

This is an intravascular prosthesis deliverable transvenously for the occlusion of large heart defects. The defects are predominantly large atrial septal defects and ventricular septal defects. The device has an occluder connected with a centering counter-occluder. The device is delivered to the heart by known methods. The occluder is released on the distal side of the defect and the proximal centering counter-occluder is stretched, pulling the device over the center of the defect. Subsequently, the centering counter-occluder is buttoned against the occluder and is opposing the right atrial side of the septum. The radiopaque button is placed upon a string to verify buttoning.

A method and apparatus for suture-less placement of an occluding patch in which release of the device can be accelerated. In one embodiment, an inactive form of an adhesive is applied on the area of the patch that will come into contact with cardiac tissue; this allows for the introduction and necessary manipulation of the catheter system until activation of the adhesive occurs. In accordance with one aspect of the invention, the adhesive properties of certain polymeric materials are relied upon rather than their ability to cure or harden into a specific shape. In another embodiment, the patch is immediately released utilizing a detaching mechanism on the balloon or the balloon catheter which supports the patch; the patch along with the inflated balloon remain on the cardiac structure occluding the opening.

This is an intravascular prothesis deliverable percutaneously with several cardiac applications including but not limited to heart valves, intravascular narrowings and cardiac defect occlusion. The device has a detachable sac connected through a thread or wire outside the body where it is immobilized. The device can remain inside the heart for as long as it is clinically useful in non permanent applications (valve, throttle); at any point it can be retrieved by pulling it outside the body by the holding wire/thread. In permanent applications (defect occlusion) the wire/thread can be withdrawn allowing the sac to endothelialize.
