Historically, healthcare services have focused on providing reactive acute illness and injury care. This model of care offers diagnosis and treatment for cure of a specific symptom or injury within a timed visit to a passive recipient of care. Unfortunately, this one-size fits all type of healthcare leaves many with unheard and/or unmet needs, missed preventive care services and opportunities to identify and modify health risks, and inadequate chronic condition care and support.
