Abstract:
The application describes a bilateral drilling and screw placement guide adapted for fixation to the spinous process of a vertebral body. The positioning and surgical guiding instrument is adapted for use during a spinal surgical procedure in conjunction with a drilling tool, fastening device, e.g. a pedicle screw, K-wire or the like. The guide includes an engagement device for attachment to a region of the spinous process, to which an adjustably attached support member is affixed. This allows for immobilizing the guide upon the spinous process anatomical landmark, in a specific orientation with respect thereto. The guide includes a bilaterally adjustable drill guide assembly for precise anatomical positioning of the drill and screw placement guides bilaterally about the sagittal, axial and coronal planes so as to enable the defining of a plurality of drilling axes extending toward the vertebral body. The device further provides for adjustment to account for anatomical variations in width along the axial plane, and includes pointing and angular positioning functionality to insure repeatable and reliable pilot hole and screw placement along a plurality of angles.

Abstract:
An intervertebral implant assembly includes an implant member and a retention member. The implant member includes a body portion and an attaching portion. The body portion includes first and second surfaces adapted to engage first and second vertebral bodies, respectively. The attaching portion defines a first guide portion that leads into the first surface. The retention member is detachably coupled to the attaching portion of the implant member. In particular, the attaching portion of the implant member is at least partially disposed in the retention member. The retention member defines an access bore configured and dimensioned to receive a bone screw therein. The access bore is in communication with the first guide portion, whereby when the bone screw is inserted through the access bore the bone screw is directed to the first vertebral body through the first guide portion.

Abstract:
The present disclosure is directed towards a biomechanical implant and anterior, lateral or posterior instrumentation construct. The construct may be of unitary or modular construction, whereby a single molded construction can form the entire assembly, in which case the through holes may be adapted to receive a metallic insert for screw fixation; or alternatively be of a modular construction wherein the anterior/lateral instrumentation and intervertebral spacer are designed for removable locking engagement, one with the other, for insertion by the surgeon as a unitary construct. A unique feature of the construct resides within the instrumentation construction, whereby a single opening formed therein permits two bone screws, or the like fastener device, to be positioned within both the superior and inferior vertebral bodies surrounding the spacer implant, or, for example in the case of a corpectomy or diskectomy with cage insertion, wherein two screws can be fixed within a single vertebral body through a single through hole, and wherein the bone screws are constructed and arranged to cooperate with the retention plate so as to provide locking engagement, one to the other, with the retention plate, upon final fixation thereof. Screw retention elements of alternative shape, based upon the choice of vertical or horizontal orientation, based upon an opened figure eight design, are provided for insertion in a groove formed in the borehole of the instrumentation plate which allows insertion of each fixation element but will prevent a loosened fixation element from falling out of the plate.
