Abstract:
Systems and methods are provided that reduce or prevent the occurrence of NIH, VR and the resulting VS in the VOT of a GAVF. The system may protect the venous outflow tract of a GAVF from the deleterious physical and biological factors that induce: pathological cellular and biochemical responses which cause neointimal hyperplasia, venous remodeling, thrombosis, venous stenosis, and GAVF-vein anastomotic stenosis, including: a fenestrated expandable stent; and a tubular extension extending from the fenestration. The method protects the venous outflow tract of a GAVF from the deleterious physical and biological factors that induce: pathological cellular and biochemical responses which cause neointimal hyperplasia, venous remodeling, thrombosis, venous stenosis, and GAVF-vein anastomotic stenosis, and include steps of inserting the device into a vein to be used as a VOT; expanding the device; and joining the tubular extension of the device to a GAVF.

Abstract:
Methods and systems are described for creating chronic vascular access or hemodialysis. These methods and systems eliminate the insertion of needles which are now required for using the available methods of obtaining vascular access. Thus multiple complications due to needle insertion through skin, tissue, and vein or graft wall are prevented. A low-profile stable transcutaneous implant's external surface and stability minimizes infection. The implant is readily joined to a connector device that functions substantially automatically and which provides high blood flow volumes for hemodialysis.

Abstract:
The system and method provide ways to achieve vascular access, e.g., for chronic hemodialysis. The system includes a port which may be bonded to a vascular graft which is installed between an artery and vein. A movable seal occludes a lumen of the port which when deployed allows access to the blood flow, allowing hemodialysis. A stent may be employed with an extension that is part of the graft. A connector may lock on to the port to deploy the seal to make a connection between the patient and a dialyzer. A cap may cover the port and seal for sterility. Methods of using the system are also disclosed.
